Supported living
Supported living is a housing and support model designed to assist individuals, often with disabilities, mental health issues or other special needs, in living as independently as possible within their community. The specific services and features included in supported living can vary
Support to access the community and attend events.
Our support workers enable the individuals we support to access the community this can be provided by various programs and organisations, including disability support services, community centres, day centres and also non profit organisations focused on improving the quality of life for individuals with learning disabilities. The services that are offered vary depending on the need of the individual and the available resources in the community. The goal is to empower individuals to lead fulfilling and active lives while being an integral part of their local community.
Personal care
Assistance with personal care refers to the support and help provided to individuals who may have difficulties performing various activities of ADLs (Activities of daily living) independently due to physical or cognitive limitations. These activities are essential for maintaining personal hygiene, health, and overall well-being. Assistance with personal care may involve the following tasks.
Bathing and showering
Toileting
Dressing
Grooming
Mobility and transfers
Medication management
Skin care
Exercise and physical therapy
During this time our support workers also provide emotional support and companionship while assisting with personal care, recognising that these tasks can be emotionally challenging for some individuals.
Access to Medical Treatment and advocating for the people we support .
Access to medical treatment for individuals with learning disabilities is a crucial aspect of ensuring their overall health and well-being are maintained. People with learning disabilities may face challenges in understanding, communicating and making decisions related to their healthcare. Meaning that our support workers provide the appropriate support and assistance ensuring that the people we support have access to the best health service’s.
They ensure that all of the information is accessible to the people we support in a format that they understand. During consultations it may be necessary for our support workers to assist with communication support such as communication boards or the use of Makaton. Care Assist promote advance care planning this will involve input from health professionals, family members and the individual, this will help ensure that a persons preferences and choices regarding medical treatment are respected in the event that they are unable to make the decision for themselves at certain times. Ensuring the individual understand what they are consenting to and knowing they have the right to decline medical treatment if they want to.
The individuals are accompanied to regular check ups, Care Assist work in a preventative way. Encouraging and facilitating regular check ups and preventative care is important to monitor and maintain the health of individuals. Continuity of care is very important it ensure a consistent and coordinated healthcare service. This is not always possible, Care Assist recognise the importance of consistency and work hard for this to be recognised throughout.
